Club Registration Checklist

To register as a Club player you will need to to do the following:

  • Please download the required forms below prior to attending your first training session and complete them and hand them to the coach prior to practice.
  • Consent to comply with COVID-19 Bio-Security Policy and Procedures found at this link;
  • The Practice Session times and Locations are HERE.
  • You need to fill in the Waiver and Fitness to Participate Form. HERE.
  • You will need to assess and decide which Volleyball NSW State Association Member Class you belong to and pay that fee. The 2021/22 Membership Schedule is and process for Individual Membership is HERE.
  • You need to join Volleyball NSW State Association for the player accident Insurance. Registration Form HERE.
  • Agree you have read and accept the Terms and Conditions of the Player Accident Insurance Policy contained in the Policy Description Statement found HERE.
  • Agree to Pay a one-off Club Registration Fee (if any);
  • Agree to Pay training fees before the commencement of each session in advance or taking to the court for training;
  • Agree to Purchase your Jersey and shorts if you play in a squad or team in a Tournament;
  • Agree to Pay your share of the team Tournament entry fee.
    • The cost will depend on which Tournament you enter.
    • The costs vary between social, State Cups, State Volleyball League and Australian Volleyball League or Social League and Scrimmage.
    • All these Tournaments have different Travel requirements, accommodation requirements and entry fee requirements so it will depend on what you enter and at what time of year.
  • Agree to provide and fill in any paper work required before you can train on the court which includes:
    • The 2022 Risk Waiver Form; and
  • Read and agree to be bound by the Clubs Policies that are found HERE.
  • Read and agree to be bound by the Clubs Code of Ethics for Players that are found HERE.
  • Read and agree with terms of the Player Accident Product Disclosure Statement and Policy Wording which is found HERE. 
  • I understand and acknowledge and agree to the following:
    • that the player accident Insurance policy does not cover all of my costs and treatment in the event of an injury.
    • I understand it is a ‘top up’ insurance after Medicare and Private Insurance is exhausted to a fixed sum.
    • Whilst the summary of cover outlines what can be considered the ‘standard’ benefits and sums insured, I have been strongly encouraged to take out Private Health, Life and Income Protection Insurance according to my own individual or my children’s circumstances.
    • I understand and agree that I am not covered by the Associations Insurance policy unless and until I become a financial member of Volleyball NSW.
